Talk about hopes, practical needs, and the questions you want to work through together, one step at a time.
Begin with separate perspectives
You and your loved one may have different hopes about what happens next. Give each person room to describe those hopes before turning them into a checklist. Ask what support is wanted, rather than assuming that taking over will help.
Turn hopes into small agreements
Pick one practical subject at a time: communication, transportation, a place to stay, daily routines, or appointments. Note what is confirmed, what needs checking, and who will follow up. If a plan depends on someone else agreeing, leave it open until they have actually agreed.
Questions to carry with you
- What matters most to each of us?
- What can we agree to now?
- What will we do if the first plan is unavailable?
One manageable next step
Make one small agreement with a person responsible for the next step and a time to revisit it. A plan can change without being a failure.
