Prepare together

Talk about hopes, practical needs, and the questions you want to work through together, one step at a time.

Keep your voice in the plan

Support works better when the people involved understand what you want to work toward. Share your priorities and ask others what they can realistically offer. You do not have to agree on every long-term decision before identifying a useful next step.

Check the practical details

For a transition, consider identification, a place to go, transportation, appointments, and ways to communicate. Ask the appropriate office which instructions apply to you. A family member’s hope or an unconfirmed offer is not yet a finished arrangement.

Questions to carry with you

  • Which arrangements have actually been confirmed?
  • Who is checking the remaining details?
  • What is our backup if a person or service is unavailable?

One manageable next step

Choose one unresolved part of the plan and identify who will confirm it. Review the plan together when circumstances change.
