What is it?

A form asks for information in labeled boxes.

Why would I need this?

Applications and appointment requests often use forms.

  1. Read the page and note which fields are required.
  2. Check that the site is the right organization.
  3. Work through one section at a time.
  4. Before Submit, review the information and note what the confirmation says. Save a receipt only when appropriate.
