Find official replacement-document instructions

I already know this — skip it

What is it?

Replacement instructions come from the agency that issues the document.

Why would I need this?

A lost card or certificate can lead you to a paid look-alike website.

Show me.

  1. Identify which agency issues the document.
  2. For a Social Security card question, begin with SSA’s official replacement-card page.
  3. For a birth certificate, find the appropriate official vital-records office.
  4. For state identification, use the official state licensing or identification agency.

Try it.

Read the official SSA link below and identify where it explains next steps. Do not enter your Social Security number on HAB.

Watch out for this.

Do not assume the same requirements apply to everyone or to every document.

Explain this another way

Go to the issuer before deciding which form you need.

Show me an example

A Social Security card and a state ID are issued through different agencies.

What does that word mean?

Application: A request to be considered for a job, housing, service, or another opportunity.

When a feature needs information, we ask only for the minimum needed to make that feature work. Our approach is to collect and retain only what the feature needs—not to build a detailed profile of you or your child. Hosting and security services may retain technical logs, such as IP addresses.
